GLP-1s and Weight Loss



GLP-1 receptor agonists are a class of medications used primarily to treat type 2 diabetes mellitus, but they also show promise in managing weight loss. Ozempic and Wegovy (semaglutide), Zepbound and Mounjaro (Trizepatide) and Saxenda (liraglutide) are among the prominent medications in this category.


How GLP-1 Works


GLP-1, or glucagon-like peptide-1, is a hormone naturally produced in the intestines in response to food intake. It plays a crucial role in regulating blood sugar levels, primarily by stimulating insulin secretion from pancreatic beta cells in a glucose-dependent manner. Here’s how GLP-1 works in more detail:


  1. Insulin Secretion: GLP-1 stimulates the release of insulin from pancreatic beta cells when blood glucose levels are elevated. This helps lower blood sugar levels after meals and promotes glucose uptake by cells throughout the body.

  2. Glucagon Inhibition: GLP-1 inhibits the release of glucagon, a hormone that typically raises blood sugar levels by stimulating the liver to release glucose into the bloodstream. By suppressing glucagon secretion, GLP-1 helps prevent excessive glucose production from the liver.

  3. Delayed Gastric Emptying: GLP-1 slows down the rate at which food moves from the stomach to the small intestine, which helps control appetite and prolongs feelings of fullness (satiety). This effect contributes to reduced food intake and can aid in weight loss.

  4. Central Nervous System Effects: GLP-1 receptors are also found in the brain, where GLP-1 can influence appetite regulation and food intake behavior. Activation of these receptors in the brain can promote satiety and reduce cravings, further supporting weight loss efforts.

  5. Beta Cell Protection and Function: GLP-1 has been shown to have protective effects on pancreatic beta cells, which are responsible for producing insulin. It can help preserve beta cell mass and function, which is beneficial in managing type 2 diabetes where beta cell dysfunction and insulin resistance are common.


In clinical practice, synthetic versions of GLP-1 called GLP-1 receptor agonists (such as Ozempic, Wegovy, Saxenda, and Zepbound) are used to mimic the actions of natural GLP-1. These medications are administered by injection and are typically prescribed to individuals with type 2 diabetes to improve blood sugar control and promote weight loss. They are effective in helping patients achieve better glycemic control, reduce insulin resistance, and manage body weight by leveraging the various mechanisms through which GLP-1 influences glucose and metabolism regulation.


Medication Options


Ozempic and Wegovy, both containing semaglutide, are injectable medications that mimic the effects of the glucagon-like peptide-1 (GLP-1) hormone. They work by stimulating insulin production in response to meals, reducing glucagon secretion, and slowing gastric emptying, which collectively help lower blood sugar levels and promote weight loss. Ozempic is typically administered once weekly, while Wegovy, approved specifically for weight management, is administered once weekly but at a higher dose.


Saxenda, another GLP-1 receptor agonist, also promotes weight loss but uses liraglutide. Like Ozempic and Wegovy, Saxenda works by enhancing insulin secretion, suppressing appetite, and delaying gastric emptying, contributing to reduced calorie intake and subsequent weight loss. It is administered daily via injection and has been approved for long-term weight management in individuals with obesity or who are overweight with weight-related comorbidities.


Zepbound, also known as tirzepatide, is a newer GLP-1 receptor agonist that combines the actions of a GLP-1 receptor agonist with those of a glucose-dependent insulinotropic polypeptide (GIP) receptor agonist. This dual action helps improve glycemic control and facilitates weight loss by multiple mechanisms, including enhancing insulin secretion and reducing appetite.


These medications offer valuable options for individuals struggling with type 2 diabetes and obesity, providing effective management of blood sugar levels and promoting significant weight loss. They are typically prescribed as part of a comprehensive treatment plan that includes lifestyle modifications such as diet and exercise to optimize health outcomes.
